Content addresses barriers to knowledge by defining, giving expert examples for conversations, and practical tips. Also, discussed an understanding and review of misunderstood perceptions and knowing the roles in determining a "Goals of Care" conversations or when to refer.

1. Recognize when to have a goals of care conversation.
2. Recognize advance directives used during goals of care meetings.
3. Assess the preparation needed prior to the goal conversation.
4. Integrate examples of who should be present for the goals of care conversation.
5. Apply the steps of a goals of care meeting starting with introduction thought making a treatment plan tailored to patient’s goals.
6. Develop outline of documentation of Goals of Care Conversations.

 

In support of improving patient care, Avera is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

 

Credit Designations

	This activity was planned by and for the healthcare team, and learners will receive 1.0 IPCE credits for learning and change.
	Medicine CME:  Avera designates this live activity for a maximum of 1.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn credit toward the CME of the American Board of Surgery’s Continuous Certification program.  It is the CME activity provider’s responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit
	Nurse: Avera designates this activity for a maximum of 1.0 ANCC contact hours. Nurses should claim only the credit commensurate with the extent of their participation in the activity.
	Nursing Facility Administrators:  Avera is an approved provider of continuing education for South Dakota through the South Dakota Board of Nursing Facility Administrators. This educational activity is approved for 1.00 hours for nursing facility administrators.
	Social Workers: As a Jointly Accredited Organization, Avera is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 1.0 General continuing education credits. *Types of credit include: ethics, clinical, cultural competence, or general.
