Overview
The vCNE Series is specially designed to update nurses working in acute care settings on hot topics, evidence based practice and Avera’s current strategic initiatives. Avera's Hospital@Home program just launched this year, bringing the next frontier of medicine and care for patients. This session will review this new tactical care strategy, including how patient referrals work for this program and what types of patients qualify.

Target Audience
Professions - Nurse - Nurse Practitioner, Nurse - RN, Nurse -LPN/LVN

ANCC Learning Outcomes
80% of nurses will pass a post test to show knowledge of a hospital-at-home program including the types of patients that qualify and how they benefit. with a score of 80% or greater. 

  1. Recognize the need to incorporate new knowledge of changes in practice into their daily care models.
  2. Clarify what a hospital at home program entails.
  3. Highlight the benefits of a hospital a home program.
  4. Highlight the types of patients that would qualify for a hospital at home program.
