Will review and describe the multiple factors that impact advance care planning and provide recommendations for next steps.

1. Give examples of who should be involved in the advance care plan conversation.
2. Outline where to find information where MOST, POLST and IPOST documents live.
3. Apply the 5 D’s to advance care planning conversations.
4. Associate the impact physicians and healthcare professionals have on completion of advance care planning
5. Define what Advance Care Planning is and is not.
