Overview
To assist organizations in implementing a sustainable CHW program, the CHWSD has partnered with Avera to develop the SD CHW Planning and Sustainability Toolkit. This toolkit was developed to be South Dakota-specific and assist medical and clinical based organizations in developing a sustainable CHW program and assisting in integrating the CHW into their own teams.

Target Audience

Professions - Dentist, Nurse - RN, Nurse -LPN/LVN, Pharmacist, Physician, Social Worker

Objectives
  1. Describe the background information, roles and responsibilities, and potential locations where CHWs work
  2. List at least two states that have established CHW workforces and describe how CHW programs have grown across the country and specifically in South Dakota
  3. List the planning steps necessary for implementing a successful CHW program
  4. Describe how to prepare your organization and community for integrating a CHW, including a needs assessment, building buy-in, and presenting the case for CHWs
  5. Identify options for funding your CHW program, including Medicaid Reimbursement of CHW services
  6. List the steps involved with implementing a CHW program in which a CHW is hired to work at an organization
  7. Prepare an organization structurally and internally for a CHW hire, using their knowledge of best practices for onboarding of CHWs, supervision, and ongoing training
  8. Compare CHW training options in line with Medicaid Reimbursement regulations
  9. Write and post a CHW job description and conduct a CHW interview
  10. Describe implementation considerations for six CHW models
  11. Describe the basic steps of evaluating your CHW program, including designing an evaluation, collecting information, and disseminating results
  12. Describe five different data collection measures for evaluating a CHW program
  13. Use the six domains of program sustainability to facilitate program sustainability planning
  14. Discuss how collaboration with CHW can lead to improved patient care and outcomes.
