Overview
The South Dakota Department of Health and the Community Health Worker Collaborative of South Dakota are both expanding the utilization of CHWs throughout the state, with the goal to offer many employment opportunities for CHWs to work in and around healthcare. Like several other states, South Dakota is expanding the use of CHWs to increase gaps in service delivery and improve quality of care. This toolkit is intended to provide you with background knowledge, resources, tips, and recommendations for hiring a CHW. This toolkit is ideal for managers or key decision makers advocating for, or implementing, a CHW program at their organization. The focus of this toolkit is on healthcare and social service settings; however, the information found in this toolkit is also applicable to community settings as they relate and interface with healthcare settings. Whether you work in a small clinic or big hospital, non-profit organization or university-based organization, CHW(s) can be a crucial part of your team.

Target Audience
Professions - EMS, Nurse - RN, Physician, Social Worker

Objectives
Outcome:  100% of nurses can self-report a knowledge gain on what the CHW role is.

  1. Describe the background information, roles and responsibilities, and potential locations where CHWs work
  2. List the planning steps necessary for implementing a successful CHW program
  3. Describe how to prepare your organization and community for integrating a CHW, including a needs assessment, building buy-in, and presenting the case for CHWs
  4. Prepare an organization structurally and internally for a CHW hire, using their knowledge of best practices for onboarding of CHWs, supervision, and ongoing training
  5. Describe the basic steps of evaluating your CHW program, including designing an evaluation, collecting information, and disseminating results
