Cancellations received 5 business days prior to the activity will be refunded the registration fee minus a $10 refund fee.  Cancellations must be received in writing to [email protected].  Following this date, no refund will be given; but substitutions will be accepted.    Refunds are issued in the same form as initial payment.

If you register to attend this conference virtually, you will be able to view presentations, hear live audio, and interact online.  This virtual activity will be fully web-based and you will need a computer/laptop or tablet, audio and an internet connection.  WebEx can be accessed from any computer - both on and off the Avera network.  Details will be emailed to all attendees before the activity.  Please be aware that you are not authorized to record, copy or share content in any manner.

The Avera Cancer Institute Oncology Symposium is designed to enhance collaboration between the provider and the multidisciplinary team in caring for individuals with cancer across the continuum of care.

1. Define tools and resources needed to manage the oncology patient in their practice setting
2. Identify evidence-based practice changes for oncology care and the impact on their day-to-day role.
3. Recognize opportunities to apply gained knowledge in the application of patient education
4. Describe how multidisciplinary roles interconnect and collaborate to provide care to the oncology patient population.

	Medicine CME: Avera designates this live activity for a maximum of 6.25 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. Successful completion of this CME activity, which includes participation in the evaluation component, enables the learner to earn credit toward the CME of the American Board of Surgery’s Continuous Certification program.  It is the CME activity provider’s responsibility to submit learner completion information to ACCME for the purpose of granting ABS credit.
	Nurse CE: Avera designates this activity for a maximum of 6.25 ANCC contact hour(s). Nurses should claim only the credit commensurate with the extent of their participation in the activity.
	Pharmacy CE: Avera designates this activity for 6.25 ACPE contact hour(s). Pharmacists and pharmacy technicians should claim only the credit commensurate with the extent of their participation in the activity. Credit will be uploaded to the NABP CPE Monitor® within 30 days after the activity completion. Per ACPE rules, Avera does not have access nor the ability to upload credits requested after the evaluation closes. It is the responsibility of the pharmacist or pharmacy technician to provide the correct information (NABP ID and DOB (MMDD)) to receive credit.
	Social Work: As a Jointly Accredited Organization, Avera is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 6.25 Clinical continuing education credit(s).
	Medical Assistant: This program has been granted prior approval by the American Association of Medical Assistants (AAMA) for 6.25 continuing education unit(s). Granting approval in no way constitutes endorsement by the AAMA of the program content or the program provider. Attendance for this activity will be submitted directly to the AAMA.
